The aim of the programme Equity in Health for Adults in Vulnerable Positions is to reduce inequity in health by maturing and scaling well-documented, patient-centred solutions that improve patient pathways for adults in vulnerable positions who live with cardiometabolic disease (CMD) as part of their multimorbidity. The Novo Nordisk Foundation has allocated a granting frame of DKK 100 million for the programme, which will be implemented over a two-year period (2025-2026).

The programme supports partnerships across sectors between civil society, general practice, municipalities and hospitals that focus on solutions that can be widely implemented as part of common practice and with documented effect for the citizens. The projects must contribute to creating better cohesion in the healthcare system and ensure that more citizens get access to relevant and timely healthcare.

The Foundation selects grant candidates by identifying relevant initiatives with promising results and scaling potential, based on desk research and dialogue with actors in the field. The candidates are invited to submit an expression of interest, and following an initial screening process, a number of the projects are then invited to submit a full application. The final applications are also submitted for external peer review before a final decision is made.

During the grant period, the projects must mature their solutions with a view to upscaling the activities. As part of the programme, the participants will be part of a learning network that helps them with implementation, knowledge sharing and anchoring of the solutions in municipalities and regions.
